Health Professional claim forms (CDF Capability)

Employers

If you are an employer and wish to claim under the CDF Approval, you must use:

Claim Form AD 138-3 (introduced August 2006)

Self-Employed Reservists

If you are a self-employed Reservist and wish to claim under the CDF Approval, you must use:

Claim Form AD 138-4 (introduced August 2006)

These claim forms should be used for all claims submitted under a CDF Approval.
